Our Mission

Our mission is to enhance the continuity of care from healthcare facility to home, increase client and caregiver safety during daily care, and maintain the client's optimal level of functional independence.

Our story began with a simple vision - to bridge the gap between healthcare facilities and home care by empowering caregivers with valuable resources that improve the lives of those we serve.

Cindy Cropped

Cindy Hardin-Weiss, MSPT, CAPS

Co-Founder

Cindy graduated in 1992 with a master's degree in Physical Therapy from Washington University in St Louis, MO. The majority of her career has been spent in home healthcare. Her passion is to assist people in maintaining their independence, especially when it makes a difference in their ability to remain in their homes.

Christina Hardin-Weiss, MS/SLP, CAPS

Co-Founder

Christina graduated from Eastern Illinois University with a master's degree in Speech/Language/Dysphagia Therapy. She has spent most of her career treating patients in skilled nursing facilities. She enjoys working with individuals who have dementia and their families, as well as those that have difficulty eating, chewing, and swallowing due to illness or disease.

Becca Meredith, OTR

Occupational Therapist

Becca graduated in 2010 from Eastern Illinois University with a bachelor's degree in Health Studies and in 2012 with a master's degree in Occupational Therapy from St. Louis University. She has practiced as an OT in skilled nursing rehabilitation centers, home health, and outpatient therapy. She enjoys addressing her patient's deficits and encouraging them to pursue what is meaningful to them.

Alex Lockridge, DPT

Physical Therapist

Dr. Alex graduated from the University of Illinois Urbana Champaign in 2005 with a bachelor of science in Kinesiology and in 2009 from the University of Maryland Baltimore with a doctorate in Physical Therapy. Her work experience includes inpatient rehabilitation, outpatient neurological rehabilitation, home health, skilled nursing, and assisted living. She is also certified in the LSVT BIG program and treats those individuals living with Parkinson's Disease.
